Waiver and Release of Liability:

1. Acknowledgement of Risk:

I, understand and acknowledge that participating in basketball training, exercises, games, and other activities at Elite Hoops Academy involves inherent risks of physical injury. These risks include, but are not limited to, sprains, fractures, overuse injuries, heat-related injuries, and in rare instances, more severe injuries such as cardiac events or catastrophic injuries.

2. Assumption of Risk

I knowingly and freely assume all such risks, both known and unknown, even those arising from the negligent acts or omissions of others, and assume full responsibility for my participation at Elite Hoops Academy™.

3.  Waiver and Release:

I hereby release, waive, discharge, and covenant not to sue Elite Hoops Academy, its officers, directors, coaches, trainers, employees, volunteers, agents, and facilities used (collectively, "Releasees") from any and all liability to me, my personal representatives, assigns, heirs, and next of kin for any and all claims, damages, demands, losses, or causes of action arising out of my participation in Elite Hoops Academy™ activities, including, but not limited to, those caused by the negligent acts or omissions of any or all of the Releasees.

4. Medical Treatment:

I give consent to Elite Hoops Academy™ and its representatives to seek emergency medical treatment for me/my child in case of injury, accident, or illness during my/my child’s participation. I understand that I will be financially responsible for any costs of such treatment.

5. Fitness to Participate:

I declare that I am physically fit and have no medical condition that would prevent my full participation in Elite Hoops Academy™ activities.

6. Media Release:

I grant Elite Hoops Academy™ the right to photograph and/or record me/my child in connection with Elite Hoops Academy™ and to use the photo and/or recording for promotional purposes.

7. Acknowledgment of Understanding:

I have read this waiver and release of liability, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
